    What each row means

    Star ratings
    CMS's overall rating starts from the health inspection rating, then moves up or down a star for staffing and for quality measures. A Special Focus Facility is not given an overall rating. How the ratings are built.
    Health citations and harm-level citations
    Every deficiency inspectors cited in the inspections on file. Harm-level citations are those rated actual harm or immediate jeopardy, the two most serious levels on CMS's grid. How to read a citation.
    Fines and payment denials
    Civil money penalties CMS imposed in the last three years, and periods when Medicare and Medicaid stopped paying for new admissions.
    Staffing hours and turnover
    Hours worked by nurses and aides per resident per day, from the payroll records each home sends CMS, and the share of nursing staff who left within a year. Each home's page shows weekend hours and a quarter by quarter history.
    Owners and chain
    The group CMS links the home to through shared owners or operators. Each home's page lists every owner and manager in the CMS ownership file.
