The facility reported an outbreak of a virus on 11/19/19 with sixteen (16) residents affected. Observations revealed nine (9) of the sixteen (16) residents' rooms did not have Personal Protective Equipment readily available outside of the room and thirteen (13) of the residents' rooms did not have signage in a conspicuous place outside the resident's room identifying the category of precautions and/or instructions to see the nurse before entering.
