Inspectors wrote, December 23, 2025, scope and severity DBased on observation, interview, and record review, the facility failed to provide two of four sampled residents (Residents 1 and 4) with enough space to maneuver their wheelchairs around their room. This deficient practice resulted in Residents 1 and 4 becoming frustrated with one another when their wheelchairs continuously bumped into one another.

Inspectors wrote, July 3, 2025, scope and severity DBased on observation and staff interview the facility failed to ensure hallways were free from clutter and allowed resident a direct access down the hallway. This was a random opportunity for discovery and had the potential to affect a minimal number of residents residing in the Long-Term Care Facility. Facility census:
