The facility failed to report alleged abuse to the ADM and HHSC on 2/11/26 at approximately 2:54 p.m. when LVN A allegedly forcefully placed an oxygen mask on Resident #1 and told her to keep it on or she would die. This failure could place residents at risk of abuse, emotional distress, and loss of dignity.

The non-compliance was identified as past non-compliance. The noncompliance began on 7/15/23 and ended on 7/16/23. The facility had corrected the noncompliance before the investigation began. This failure could place residents at risk for inaccurate drug administration resulting in decline in health and decreased quality of life.
