Resident #11 was transferred to the hospital on [DATE] and Resident #51 was transferred to the hospital on [DATE]. There was no evidence that Resident #11's or Resident #51's responsible party was given written notification of the bed hold and the reserve bed payment policy upon transfer to the hospital.
