Findings Include: On 6/11/18 at 4:49 p.m., the survey result note book was observed in the front lobby of the facility. The notebooks contained results from the April 2017 recertification survey and life safety survey. There was no notice posting the availability of the previous three years of survey results. Employee Identifier (EI) # 2, Social Service Director, was interviewed on 6/12/18 at 3:25 p.m. EI # 2 stated the survey results were posted in the front lobby.
