The facility failed to implement its infection prevention and control policies and procedures and identify and correct problems relating to infection prevention practices for 4 out of 14 sampled residents, Resident (R) 4, R11, R40, and R44. The deficient practice had the potential to affect the whole facility with a census of 28. The failures were related to improper cleaning of the residents' shared glucometers, improper hand hygiene, and improper handling of resident's medication.
