Inspectors wroteBased on record review and resident and staff interviews, the facility failed to provide care in a safe manner when Resident #4 rolled off the left side of her bed during incontinence care and required transfer to the hospital for medical evaluation. This was for 1 of 4 residents reviewed for accidents (Resident #4).
