Inspectors wrote, August 7, 2026, scope and severity EBased on review of facility policy, clinical records, and staff interview, it was determined that the facility failed to provide evidence medication regimen reviews (MRRs) were reviewed by the resident's attending physician monthly for three of five residents (Residents R2, R4 and R7).

Providence Health & Rehab Center, Beaver Falls, PA

Inspectors wrote, August 5, 2026, scope and severity DBased on observation, record review, and interview, the facility failed to ensure the Consultant Pharmacist (CP) identified and reported when Resident (R) 1's blood glucose (the main sugar in your blood and your body's primary energy source) level was below the physician's ordered parameter and the physician ordered insulin (a hormone that lowers the level of glucose in the blood) was not held. The facility failed to ensure the CP identified and reported when R27's physician ordered parameters for blood pressure and/or pulse were not obtained and recorded prior to the administration of his ordered antihypertensive (a class of medication used to treat high blood pressure) medications.

Inspectors wrote, July 28, 2026, scope and severity DBased on record review, policy review, and staff interviews, it was determined that the facility failed to: (1) consistently maintain the required separate written reports of medication regimen review irregularities, and (2) ensure documentation was available to demonstrate that pharmacist-identified medication regimen review irregularities had been reviewed by the physician and that the physician had responded to those irregularities. This deficient practice was evident for 2 (Residents #2 and #5) of 5 residents reviewed for Medication Regimen Review during the facility's recertification survey.
