This was evident for thirteen (Residents #2, #10, #11, #12, #13, #14, #15, #16, #17, #18, #19, #21, and #24) of the fourteen residents interviewed regarding this change.

Elkton Nursing and Rehabilitation Center, Elkton, MD

Inspectors wrote, July 10, 2026, scope and severity EBased on interview and record review, the facility failed to respect the rights of five of eight sampled residents (Residents 1, 2, 3, 4, and 5) by:1. Failing to ensure Resident 1 and Resident 2's rights to choose their own attending physician (AP) were followed.2. Failing to ensure Residents 3, 4, and 5 were given an option to choose their own AP while at the facility. These failures violated the rights of Residents 1, 2, 3, 4, and 5 in choosing their own AP while at the facility.

Inspectors wrote, June 29, 2026, scope and severity DBased on observations, interviews, and record review, the facility failed to honor the residents' rights to choose their attending physician for 1 (#57) of 23 sampled residents. This deficient practice affected all residents wanting to choose their primary physician and resulted in resident #57 feeling she could not use her primary physician she had built a relationship with and was comfortable using.
