The deficient practice resulted in residents not having a dining area that accommodates residents' physical needs. The census was 79.

Inspectors wrote, June 26, 2026, scope and severity FBased on observation, interview and record review, the facility failed to ensure dining rooms included enough space for residents to dine, and for residents to enter and exit a dining room without having to move other residents in 1 of 1 dining rooms. This failure created risk for the inability of staff to respond quickly in an emergency situation, and risk for decreased quality of life for residents unable to dine with their peers.
