Inspectors wroteBased on interview, record review, and review of the Center for Medicare and Medicaid Services Memo QSO 20-38 -NH, the facility failed to recognize a COVID outbreak and implement Centers for Disease Control (CDC) guidance regarding: 1.) the testing of all residents and staff when a resident and/or staff test positive for COVID; and 2.) enhanced isolation and Personal Protective Equipment Precautions for residents and staff. Specifically, Certified Nurse Aide (CNA)1 tested positive for COVID-19 on 07/11/22 and Resident (R)153 tested positive for COVID on 07/14/22. The census at the time of the survey was 109.
