What inspectors wrote

Three recent F730 citations from different states, quoted from the inspectors' statements of deficiencies (CMS form 2567). Names are removed by CMS.

Inspectors wrote, August 7, 2026, scope and severity EBased on review of personnel records and staff interview, it was determined that the facility failed to complete annual performance evaluations at least once every 12 months for three of three nurse aide (NA) personnel records (NA Employee E23, NA Employee E24, and NA Employee E25).

Providence Health & Rehab Center, Beaver Falls, PA

Inspectors wrote, July 28, 2026, scope and severity FBased on interview and record review, the facility failed to conduct performance reviews for 5 of 5 Certified Nursing Assistants (CNA's) (Staff M, N, O, P, & Q) at least every 12 months, as required. This failure placed residents at risk of receiving care from staff whose performance did not meet expectations and required additional training.

North Auburn Care, Auburn, WA

Inspectors wrote, July 28, 2026, scope and severity DBased on interviews and record reviews, the facility failed to complete an annual performance evaluation (a regular check-in where a company looks at how well an employee does their job) for Certified Nurse Assistant (CNA) 6 who was facing allegations of abuse from one out of five sampled residents (Resident 8) and complaints from Resident 11's family member (FM 1). The failure to conduct a performance evaluation on an employee facing complaints places the residents at risk for compromised safety, poor service and poor quality of care which can be detrimental to the residents' health.
