Inspectors wrote, July 29, 2026, scope and severity FBased on interview and record the facility failed to respond to resident call lights (primary purpose is to allow patients to request immediate assistance from nursing or care staff. It serves as a vital safety tool used for pain medication requests, toileting assistance, personal help, or alerting staff to medical emergencies) in a timely manner for 11 of 11 sampled residents (Resident 1, Resident 2, Resident 3, Resident 4, Resident 5, Resident 6, Resident 7, Resident 8, Resident 9, Resident 10, and Resident 11). This failure had the potential for care needs to not be met, injuries to occur, and other negative outcomes.

Inspectors wrote, July 29, 2026, scope and severity DBased on observation, interview, and record review, the facility failed to ensure residents could call for staff assistance through a communication system which relays the call directly to a staff member or to a centralized staff work area from each resident's bedside for 1 of 6 residents (Resident #42) reviewed for resident call system. The facility failed to ensure Resident #42 had a call light button functioning at bedside. This failure could place residents at risk of a delay in assistance and decreased quality of life, self-worth, and dignity.

Marshall Manor Nursing & Rehabilitation Center, Marshall, TX

Inspectors wrote, July 27, 2026, scope and severity FBased on interviews, record review and facility policy review, the facility failed to have an adequate functioning call light system in place and staff did not follow the facility plan while the call light system was down by providing adequate resident supervision documented with attestation statements. This affected nine residents (Residents #326, #334, #335, #340, #341, #342, #348, #354 and #355) and had the potential to affect all residents residing in the facility. The facility census was 119.
