Inspectors wrote, August 7, 2026, scope and severity DBased on review of facility in-service documentation, personnel files, and staff interviews, it was determined that the facility failed to provide training on Quality Assurance and Performance Improvement (QAPI) for one of five staff members (Registered Nurse (RN) Employee E22).
