Health inspections
Federal rules call for a standard inspection at least every 15 months, plus a visit whenever a complaint is filed. Each mark below is one citation, colored by how serious inspectors rated it. How to read a citation.
Under each citation, the quoted text is the inspector's own summary from the federal statement of deficiencies (CMS form 2567), word for word apart from a privacy scrub; CMS removes residents' and staff names before it publishes the text. The full notes are on Medicare.gov.
Where its citations fall on CMS's grid
CMS rates every citation by how much harm it caused (rows) and how many residents it touched (columns). The count in each box is this home's, across all 8 health citations on file.

- F Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Inspectors wroteBased on observations, interviews and record review, the facility failed to follow proper sanitation and food handling practices by failing to ensure the kitchen's ice machine was clean and free of black biofilm. This had the potential to negatively affect all 94 residents currently residing in the facility.
- D Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Inspectors wroteBased on observation, record review and interviews, the facility failed to implement a care plan for one (Resident #5) of 38 residents in the total survey sample for administration of medications as ordered by the physician. The nursing staff did not adhere to the parameters set by the resident's physician, which could lead to a dangerous cardiovascular event such as severe hypotension (low blood pressure).

This could have a potentially negative affect on 65 of the 82 residents currently in the facility who actively receive Medicare/Medicaid.

- D Provide and implement an infection prevention and control program.
Inspectors wroteBased on observations, staff interview, medical record review, and a review of facility policies, the facility failed to maintain infection prevention and control standards for one (Resident #538) of 31 residents in the total survey sample, by leaving the resident's enteral nutrition line uncapped and open to air when it was not in use. The facility also failed to maintain infection prevention and control standards when one (Licensed Practical Nurse (LPN) A) of six nurses observed during medication administration failed to clean/disinfect a blood pressure cuff between resident uses. This practice could negatively impact any resident having their blood pressure checked by this nurse using this cuff. Written standards, policies, and procedures must include standard and transmission-based precautions to be followed to prevent the spread of infections. [...]
