    Inspectors wrote2. An Indiana Department of Health Intake Information report indicated Resident B had been dropped after a staff member improperly secured him to a lift. The clinical record for Resident B was reviewed on 1/16/24 at 11:23 a.m. The diagnoses included, but were not limited to, paroxysmal atrial fibrillation, type 2 diabetes mellitus with neuropathy, idiopathic autonomic neuropathy, and ataxic gait. An admission Minimum Data Set (MDS) assessment, dated 7/28/23, indicated the resident did not experience cognitive impairment. Section GG (functional abilities and goals) indicated the resident required partial/moderate assistance with toilet transfers and the helper did less than half of the effort. A nursing progress note, dated 8/12/23 at 10:00 a.m., indicated the CNA 3 notified LPN 2 the resident was on the floor in his bathroom. [...]
