Resident #2 required assist of 1 staff with a gait belt (a belt placed around the waist to aide in safe transfers and ambulation) for transfers and ambulation. A Certified Nurse Assistant (CNA) let go of the gait belt resulting in Resident #2 falling and fracturing her hip. Resident #1 was to not be left unattended in his room unless he was in bed. Resident #1 was found on floor unattended at least twice in his room. The facility reported a census of 47 residents.
