F558 citations by state

Counts depend on how many homes a state has and how often its inspectors visit, so a larger number is not by itself a worse record.

By inspection year: 2017 1, 2018 17, 2019 86, 2020 43, 2021 134, 2022 267, 2023 769, 2024 1,400, 2025 1,200, 2026 712.

How often is F558 cited?
The CMS files as of September 1, 2026 list 4,629 F558 citations at 3,437 nursing homes, 974 of them in the 12 months to that date. 0.4% were rated as actual harm or immediate jeopardy.
Is a F558 citation serious?
It depends on the letter. Most F558 citations (99.1%) were rated as potential for more than minimal harm, D to F. None in the current files is at the immediate jeopardy level.
