Inspectors wrote, August 6, 2026, scope and severity DBased on observation and interview, the facility failed to ensure the most up to date Nursing Home Resident [NAME] of Rights (RBOR) was provided to each resident residing in the facility and displayed for residents, visitors, and staff to review. This had the potential to affect all 71 residents currently residing in the facility as well as all staff and visitors.

Inspectors wrote, July 29, 2026, scope and severity DBased on record review and interview, the facility failed to ensure 1 resident (#12), out of 1 resident reviewed, was informed, orally and in writing, of the resident's rights upon admission. The facility was unable to demonstrate that the resident received the required notice of resident rights and services or that receipt of the information was acknowledged in writing. This failed practice had the potential to affect Resident #12's awareness of the resident's rights and responsibilities during the stay in the facility.

Inspectors wrote, July 24, 2026, scope and severity DBased on review of facility documentation and staff interviews it was determined that the facility failed to inform the resident both orally and in writing in a language that the resident understands of his or her rights and all rules and regulations governing resident conduct and responsibilities during the stay in the facility for one of 22 residents reviewed (Resident R102).
