Inspectors wrote, March 19, 2026, scope and severity EBased on interview and record review, the facility failed to maintain record of successful completion of a State-approved paid feeding assistant training course for 6 of 12 paid feeding assistants. Review of the lists provided by the facility, the facility had 12 staff who were Paid Feeding Assistants and 9 residents who were approved for the Paid Feeding Assistant program. In an interview on 03/19/26 at 12:55 PM, Director of Nursing (DON) B reported the facility was only able to locate the documentation of completion of a State-approved paid feeding assistant training course for 6 of the 12 staff who worked as Paid Feeding Assistants. Inspectors wrote, August 15, 2025, scope and severity DBased on observation, record review and interviews during the recertification survey 8/11/25-8/15/25, the facility did not ensure individuals working in the facility under the title Home Health Aides, had successfully completed a State-approved training program for feeding residents for 12 of 12 employees reviewed (Home Health Aides #4, #8, #32, #33, #34, #35, #36, #37, #38, #39, #40 and #41.) Specifically, there was no documentation the Home Health Aides received the required training to feed residents and Home Health Aide # 4 was observed feeding Residents on more than one occasion and Home Health Aide#38 and #36 were observed feeding residents on the Dementia Unit.
