Inspectors wrote, July 30, 2026, scope and severity DBased on interview and record review, the facility failed to ensure an electrocardiogram (EKG- a test that measures the electrical activity of the heart) that was ordered by a physician on 6/20/2026 was completed for one of three sampled residents (Resident 1). This deficient practice resulted in the delay of necessary care and services for Resident 1.

Stoney Point Healthcare Center, Chatsworth, CA

Inspectors wrote, May 11, 2026, scope and severity DBased on interview, record review and review of other facility documentation, it was determined that for one (R9) out of four residents reviewed for change in condition, the facility failed to ensure that a stat (at once) ordered chest x-ray was completed until 31 hours after it was ordered. In addition, the facility failed to ensure that R9's STAT chest x-ray result was reported to the physician in a timely manner.
