The facility's administration failure to ensure effective use of resources to ensure residents safety and prevent neglect created a likelihood of avoidable falls and accidents from mechanical lifts with a likelihood of serious injury, impairment or death of residents who use mechanical lifts and resulted in the determination of Immediate Jeopardy.
