Inspectors wrote, August 5, 2026, scope and severity DBased on observation, policy review, record review, and staff interview, it was determined the facility failed to ensure resident's right were protected to be free from abuse. This was true for 1 of 1 resident (Resident #56) observed during lunch assistive feeding. This failure placed all residents at risk for potential abuse and potential physical and psychosocial harm.

Mini-Cassia Care Center, Burley, ID

Inspectors wrote, August 5, 2026, scope and severity DBased on clinical record review, staff interviews, facility documentation, and review of applicable policies and procedures, the facility failed to ensure two of five sampled resident's (Resident #75 and Resident #52) right to be free from physical abuse by another resident (Resident #43 and Resident #6). The deficient practice had the potential to result in additional incidents of abuse and failure to take appropriate measures to protect the residents.

Haven of Safford, Safford, AZ

Inspectors wrote, July 31, 2026, scope and severity GBased on a review of clinical records, facility policy, facility-provided investigative documentation, and staff interviews, it was determined the facility failed to protect one of 23 residents reviewed (Resident 110) from neglect by failing to ensure staff implemented the resident's individualized care plan requiring the assistance of two staff members for transfers resulting in actual harm in the form of a fracture of the right distal tibia and fibula (the two bones of the lower leg located between the knee and ankle) that required hospital evaluation and treatment. This deficient practice is cited as past noncompliance.
