F726 citations by state

Counts depend on how many homes a state has and how often its inspectors visit, so a larger number is not by itself a worse record.

By inspection year: 2018 7, 2019 45, 2020 19, 2021 51, 2022 142, 2023 403, 2024 798, 2025 649, 2026 344.

Common questions

What is F726 in a nursing home inspection?
F726 is the federal requirement to "ensure that nurses and nurse aides have the appropriate competencies to care for every resident in a way that maximizes each resident's well being". CMS files it under nursing and physician services. A home cited under F726 did not meet that requirement at the inspection.
How often is F726 cited?
The CMS files as of September 1, 2026 list 2,458 F726 citations at 2,054 nursing homes, 493 of them in the 12 months to that date. 9.2% were rated as actual harm or immediate jeopardy.
Is a F726 citation serious?
It depends on the letter. Most F726 citations (90.6%) were rated as potential for more than minimal harm, D to F. 162 were immediate jeopardy, the most serious level.
