F700 citations by state

Counts depend on how many homes a state has and how often its inspectors visit, so a larger number is not by itself a worse record.

By inspection year: 2018 6, 2019 32, 2020 24, 2021 53, 2022 131, 2023 281, 2024 519, 2025 422, 2026 193.

Common questions

What is F700 in a nursing home inspection?
F700 is the federal requirement to "try different approaches before using a bed rail. if a bed rail is needed, the facility must (1) assess a resident for safety risk; (2) review these risks and benefits with the resident/representative; (3) get informed consent; and (4) correctly install and maintain the bed rail". CMS files it under quality of life and care. A home cited under F700 did not meet that requirement at the inspection.
How often is F700 cited?
The CMS files as of September 1, 2026 list 1,661 F700 citations at 1,490 nursing homes, 279 of them in the 12 months to that date. 2.6% were rated as actual harm or immediate jeopardy.
Is a F700 citation serious?
It depends on the letter. Most F700 citations (97.3%) were rated as potential for more than minimal harm, D to F. 38 were immediate jeopardy, the most serious level.

The interpretive guidance inspectors use for each F-tag is CMS's State Operations Manual, Appendix PP. NursingHomeClear reports the citations as CMS records them and is not affiliated with CMS. All F-tags · Methodology
