F693 citations by state

Counts depend on how many homes a state has and how often its inspectors visit, so a larger number is not by itself a worse record.

By inspection year: 2018 12, 2019 65, 2020 19, 2021 68, 2022 179, 2023 529, 2024 855, 2025 759, 2026 487.

Common questions

What is F693 in a nursing home inspection?
F693 is the federal requirement to "ensure that feeding tubes are not used unless there is a medical reason and the resident agrees; and provide appropriate care for a resident with a feeding tube". CMS files it under quality of life and care. A home cited under F693 did not meet that requirement at the inspection.
How often is F693 cited?
The CMS files as of September 1, 2026 list 2,973 F693 citations at 2,480 nursing homes, 677 of them in the 12 months to that date. 2.5% were rated as actual harm or immediate jeopardy.
Is a F693 citation serious?
It depends on the letter. Most F693 citations (97.3%) were rated as potential for more than minimal harm, D to F. 27 were immediate jeopardy, the most serious level.
