Inspectors wrote, July 30, 2026, scope and severity EBased on review of clinical records, as well as resident interviews and staff interviews, it was determined that the facility failed to ensure that residents were provided appropriately fitting incontinence products for three of 11 residents reviewed (Residents 1, 4, 11).

Inspectors wrote, July 18, 2026, scope and severity EBased on observation, interview and record review, the facility failed to ensure resident rooms were at a comfortable temperature for three (3) of 3 sampled residents (Resident 1, Resident 2 and Resident 3) by failing to ensure the facility's Heating, Ventilation and Air Conditioning (HVAC, a climate control system that keeps indoor areas at a comfortable temperature) was in good repair. This deficient practice had the potential to result in Residents 1, 2 and 3 feeling uncomfortably hot and negatively affect their quality of life.

Inspectors wrote, April 30, 2026, scope and severity DBased on interview and record review, the facility failed to answer a call light in a timely manner for 1 of 3 sampled residents (Resident 1) reviewed for call light response time.
