By inspection year: 2018 6, 2019 40, 2020 7, 2021 46, 2022 91, 2023 326, 2024 570, 2025 482, 2026 274.

Common questions

What is F925 in a nursing home inspection?
F925 is the federal requirement to "make sure there is a pest control program to prevent/deal with mice, insects, or other pests". CMS files it under environmental. A home cited under F925 did not meet that requirement at the inspection.
How often is F925 cited?
The CMS files as of September 1, 2026 list 1,842 F925 citations at 1,484 nursing homes, 394 of them in the 12 months to that date. 1.5% were rated as actual harm or immediate jeopardy.
Is a F925 citation serious?
It depends on the letter. Most F925 citations (97.6%) were rated as potential for more than minimal harm, D to F. 19 were immediate jeopardy, the most serious level.

The interpretive guidance inspectors use for each F-tag is CMS's State Operations Manual, Appendix PP. NursingHomeClear reports the citations as CMS records them and is not affiliated with CMS. All F-tags · Methodology
