(Lunch meal 8/4/26) The facility failed to follow the week 5 menu for pureed lunch meals served at the facility on Tuesday 8/4/26 by not serving or substituting all food items listed on the menu. This failure could place residents that eat food from the kitchen at risk of poor intake, and/or weight loss.

Inspectors wrote, July 31, 2026, scope and severity DBased on observations, interviews, and record review, the facility failed to serve the dietician approved pureed menu for 1 of 2 meals observed for staff following the correct menu. This deficient practice was identified for 2 (Resident #R4, Resident #R8) of the 5 residents who had orders for a pureed diet.
