F745 citations by state

Counts depend on how many homes a state has and how often its inspectors visit, so a larger number is not by itself a worse record.

By inspection year: 2018 2, 2019 16, 2020 3, 2021 14, 2022 31, 2023 134, 2024 267, 2025 189, 2026 121.

Common questions

What is F745 in a nursing home inspection?
F745 is the federal requirement to "provide medically-related social services to help each resident achieve the highest possible quality of life". CMS files it under quality of life and care. A home cited under F745 did not meet that requirement at the inspection.
How often is F745 cited?
The CMS files as of September 1, 2026 list 777 F745 citations at 702 nursing homes, 166 of them in the 12 months to that date. 4.5% were rated as actual harm or immediate jeopardy.
Is a F745 citation serious?
It depends on the letter. Most F745 citations (95.4%) were rated as potential for more than minimal harm, D to F. 9 were immediate jeopardy, the most serious level.

The interpretive guidance inspectors use for each F-tag is CMS's State Operations Manual, Appendix PP. NursingHomeClear reports the citations as CMS records them and is not affiliated with CMS. All F-tags · Methodology
