What inspectors wrote

Three recent F580 citations from different states, quoted from the inspectors' statements of deficiencies (CMS form 2567). Names are removed by CMS.

Inspectors wrote, August 12, 2026, scope and severity DBased on observation, interview, and record review, the facility failed to notify residents' physicians and/or Resident Representatives for three (Resident #2, Resident #37 and Resident #28) of ten residents reviewed for skin issues, one (Resident #11) of three reviewed for ADL (Activities of Daily Living) care, and one (Resident #30) of three reviewed for accidents.

Lake City Healthcare and Rehabilitation Center, Lake City, FL

Inspectors wrote, August 5, 2026, scope and severity DBased on record review, observation, and interviews, the facility failed to ensure Resident (R) 39's legal representative was updated with changes to R39's treatments or medications.

North Point Skilled Nursing Center, Paola, KS

Inspectors wrote, August 5, 2026, scope and severity DBased on record review, staff interview, and policy review, it was determined the facility failed to ensure residents' representatives were immediately notified when residents fell or had a change in condition. This was true for 2 of 4 Residents (#7 and #57) whose records were reviewed for changes in condition. This deficient practice placed residents at risk of harm due to lack of advocacy and support from their representatives when they were unable to make decisions for themselves due to decreased health status and level of consciousness.

F580 citations by state

Counts depend on how many homes a state has and how often its inspectors visit, so a larger number is not by itself a worse record.

By inspection year: 2018 26, 2019 128, 2020 38, 2021 123, 2022 260, 2023 931, 2024 1,844, 2025 1,608, 2026 974.

Common questions

What is F580 in a nursing home inspection?
F580 is the federal requirement to "immediately tell the resident, the resident's doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident". CMS files it under resident rights. A home cited under F580 did not meet that requirement at the inspection.
How often is F580 cited?
The CMS files as of September 1, 2026 list 5,932 F580 citations at 4,303 nursing homes, 1,402 of them in the 12 months to that date. 7.5% were rated as actual harm or immediate jeopardy.
Is a F580 citation serious?
It depends on the letter. Most F580 citations (92.2%) were rated as potential for more than minimal harm, D to F. 264 were immediate jeopardy, the most serious level.

The interpretive guidance inspectors use for each F-tag is CMS's State Operations Manual, Appendix PP. NursingHomeClear reports the citations as CMS records them and is not affiliated with CMS. All F-tags · Methodology
